Registration
We are based within St Andrews Community Hospital, Largo Road, St Andrews and cover the following areas: Balmullo, Blebo Craigs, Boarhills, Ceres, Dairsie, Dunino, Guardbridge, Kingsbarns, Largoward, Leuchars, Peat inn, Strathkinnes and St Andrews. Look at a map of our practice boundary. We are unable to register patients who live out with the practice boundary. We do not register patients who work in St Andrews but live out with the boundary.
You can register in person at the practice by bringing in your NHS card where you will also be asked to complete a registration form. Alternatively download a Patient Registration Form for completion and drop it in to the practice.
All prospective patients are also asked to complete a Medical Questionnaire, which you can also download and complete in advance - visit the Medical Questionnaire page to choose the correct download. You are cordially invited to attend for an initial Health check with a member of our nursing team.
If you are a new patient form overseas and have not previously been registered with a GP in the UK, we require to see your passport or National Identity card. We also require to see proof of your current address within our practice area; for example a utility bill.
