If you are NOT a student at the University of St Andrews please download and complete the Standard New Patient Medical Questionnaire Form
New Student Patient
If you are a student studying at the University of St Andrews and registered as a patient at the Strathcairn Medical Practice download and complete the New Student Patient Medical Questionnaire Form.
New Child Patient
If you are registering a child under the age of 13 please download and complete the New Child Patient Questionnaire Form.
